Speech-Language Pathologist (SLP) – Home Health

Starlight Home Health is seeking a compassionate and dependable Speech-Language Pathologist (SLP) to provide therapy services to patients in their homes. This is a great opportunity for an SLP who enjoys working independently while making a meaningful difference in patients’ safety, communication, and quality of life.

Responsibilities

Complete speech therapy evaluations and reassessments

Develop individualized plans of care and treatment goals

Provide treatment for speech, language, cognitive-communication, and swallowing disorders

Provide dysphagia assessment, treatment, and education as appropriate

Educate patients, families, and caregivers

Communicate changes in condition and recommendations to providers and the interdisciplinary team

Complete accurate and timely home health documentation

Coordinate care with nursing, PT, OT, providers, and other members of the care team

Qualifications

Current Colorado Speech-Language Pathologist license

Graduate degree in Speech-Language Pathology from an accredited program

Current CPR/BLS certification

Valid driver's license, reliable transportation, and willingness to travel to patients' homes

Strong communication and documentation skills

Home health experience preferred, but not required
