RN Health Coach - Direct-to-Consumer Care & Benefits

Florida Blue seeks a Nurse Health Coach to join its Direct-to-Consumer team in Florida. You will engage with prospective customers and members, assess needs, educate on benefit plans, costs, choices, tools and programs, and guide them through the care continuum in a Florida Blue Center environment. Duties include Personal Health Assessments, educating on biometric results, closing care gaps, presenting health education classes, coordinating care plans, and referrals to community resources. #J-18808-Ljbffr WEB-TPA, Inc.
