Job Details Schedule: Monday-Friday, 8:30 AM-5:00 PM Location: Newburgh, NY Job Type: Travel contract / Local Contract Contract Length: 13 Pay Range: $52 to $58/ hr on W2 *Pay offered is based on experience,expertise, credentialing, and education. Duties Provide skilled nursing care to patients in their homes. Perform comprehensive patient assessments and identify clinical needs. Develop and implement individualized plans of care. Monitor patient conditions and communicate changes to physicians and the care team. Administer medications and provide treatments as ordered. Educate patients and families regarding medications, disease management, safety, and self-care. Complete accurate and timely clinical documentation. Coordinate care with physicians, therapists, aides, and other healthcare professionals. Participate in case conferences and care coordination activities. Meet the 30-point weekly productivity expectation . Participate in required weeknight and weekend on-call coverage.
