• Provide direct patient care to residents in our skilled nursing facility, including administering medications, wound care, and vital signs monitoring

• Develop and implement individualized care plans in collaboration with the interdisciplinary care team

• Maintain accurate and comprehensive records of patient care and services

• Assist with daily living activities, such as bathing, dressing, and feeding, as needed

• Participate in quality improvement initiatives and contribute to a positive and supportive work environment
