We will take the time to tailor individual personalized treatment plans and set achievable goals.Under the direct supervision of the Director of Behavior Health Service, we are seeking a full-time Licensed Clinical Social Worker to provide mental health therapy for patients its Outpatient Behavioral Health Program.Essential Duties:Ability to work across the age spectrum and working with families.Performs Mental Health Intakes for patients to ascertain their diagnoses, determine appropriate level of care, and establish treatment planProvides ongoing individual and/or group therapy, maintaining ultimate responsibility for directing the course of the patient's mental health care and treatment plan within the scope of their expertiseProvides crisis intervention to diffuse conflicts among patientsEnsures a welcoming, unbiased, and inclusive environment for all who seek careCompletes medical forms and documentation related to patient care including any necessary to facilitate transfer, and if indicated, referrals to outside programs or psychiatric clinicsEncourages health promotion, disease prevention, and health maintenanceProvides patient advocacy, as appropriate for the siteCompletes documentation including diagnosis, treatment, and billing, thoroughly, accurately, and in a timely fashionMakes and/or tracks all referrals to other mental health agencies and providersWorks collaboratively with specialists for consultation and referral purposes, collaborates with psychiatric providers and medical providers within and across the clinic as neededMaintains confidential case files, including documenting requests for confidential informationFollows the clinical chart review policies and proceduresMay provide workshops for families on communication and other family-related issuesCoordinates emergency responses, including a team to minimize patient's danger to self and others, direct discussion with EMS/FDNY regarding patient's condition, referral to psychiatric or medical emergency departments and follow-up while patient is in the emergency department or upon releaseParticipates in case-conference and case management activitiesUtilizes supervision effectively to improve patient care; engages in critical reflection to develop personally and professionallyRequirements: Background and Education:Minimum of two years of clinical experience.Fluency in English and Spanish is preferred.Possess a valid, unrestricted New York State Licensed Clinical Social Worker license.Prior experience with substance abuse issues preferred.Substantive knowledge of EMR systems, Athena is preferred.Compensation details: 80000-80000 Yearly SalaryPId37a08dbdc6a-25405-41069585
