Job Summary The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Responsibilities Complete initial, comprehensive and ongoing assessments of patient and family to determine hospice needs. Provide a complete physical assessment and history of current and previous illnesses. Provide professional nursing care by utilizing all elements of the nursing process. Assess and evaluate patient status, writing and initiating plans of care. Regularly re‑evaluate patient and family/caregiver needs and revise plan of care as necessary. Use health assessment data to determine nursing diagnoses. Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions. Include the patient and family in the planning process. Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician in the physician’s plan of care. Counsel the patient and family in meeting nursing and related needs. Provide health care instructions to the patient as appropriate per assessment and plan. Develop and initiate the home‑health plan of care and assume responsibility for ongoing interdisciplinary assessment and individualized plan development in partnership with patient, representative (if any) and caregiver(s). Initiate proper preventive and rehabilitative nursing procedures and provide services ordered by the physician as indicated in the plan of care. Provide patient, caregiver and family counseling. Provide patient and caregiver education. Prepare and update clinical notes. Communicate with the physician responsible for the home‑health plan of care and other health‑care practitioners related to the current plan. Identify discharge‑planning needs as part of the care plan and implement prior to discharge. Communicate changes in the patient’s condition and obtain or receive physician’s orders as required. Coordinate with community health–related persons to align the care plan. Participate in the HHA’s quality assessment and performance‑improvement program and in‑service training. Participate in on‑call duties as defined by the on‑call policy. Ensure arrangements for equipment and other necessary items and services are available. Instruct, supervise and evaluate home‑health aide care every two weeks. Supervise Licensed Practical Nurses (LPNs)/Licensed Vocational Nurses (LVNs) and Home‑Health Aides (HHAs)/Certified Nursing Assistants (CNAs) as directed. Supervise ancillary personnel and delegate responsibilities when required. Maintain and upgrade professional knowledge and practice skills through continuing education and in‑service classes. Meet requested and/or accepted case assignments. Actively participate in quality‑assessment and performance‑improvement teams and activities. Qualifications Graduate of a National League for Nursing accredited school of nursing. Minimum of two years of experience, at least one in public health, home care or hospice nursing (preferred). Registered nurse with a current license to practice professional nursing in the state. Demonstrated excellent observation, problem‑solving, verbal and written communication and nursing skills per competency checklist. Ability to organize and prioritize workload independently. Capability of prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients and supplies. Considerable reaching, stooping, bending, kneeling, and/or crouching. Adequate visual acuity and hearing for required nursing skills. Valid current CPR certification. Valid driver’s license, automobile insurance coverage, and access to an automobile for agency‑related travel. Self‑directed and able to work with minimal supervision.
