Occupational Therapy - OT SNF
This position is for an Occupational Therapist (OT) working MondayFriday from 8:00 AM 4:30 PM, averaging 35 hours per week with no weekend rotation. The OT provides rehabilitation services in a 126-bed skilled nursing facility, helping patients recover from illness, injury, or surgery. The role focuses on improving independence with daily activities, mobility, and overall functional ability through individualized therapy programs.
The Occupational Therapist will perform patient evaluations, develop individualized treatment plans, and provide therapy interventions including ADL training, therapeutic exercises, coordination and mobility activities, and adaptive equipment education. The OT will monitor patient progress, update care plans as needed, document therapy sessions, and collaborate with interdisciplinary teams to support patient recovery and safe discharge planning.
