Job: Occupational Therapist (OT) Home Health - Part Time per Diem Location: Fremont, Newark, Union City ABL Health Care provides home healthcare, dedicated to delivering compassionate, personalized care that prioritizes comfort, dignity, and support for patients and families. Position Overview We seek an experienced, self-driven Occupational Therapist (OT) to provide exceptional care throughout the region. The OT will evaluate patients' health, create and implement care plans, and track progress, working closely with the Clinical Director and staff, both remotely and in the field. Regularly evaluate and re-evaluate patient status. Administer treatments, monitor patient conditions, and report signs/symptoms indicating a change to Physician, Case Manager, or Director of Nursing. Initiate, develop, implement, and revise the patient's plan of care (POC). Address all problems identified in the POC. Provide educational information to the patient/family/caregiver regarding knowledge deficits or needs about the patient's condition. Communicate with other disciplines involved in the case as warranted. Maintain accurate clinical records and submit documentation per company policy using a company-provided device. Comply with all company policies and procedures to ensure high standards of care. Requirements Current OT License to practice in California (Required). Experience in Home Health or Hospice care – 1 year required. Current California Driver's License. Able to travel within a 15–25 mile radius regularly. #J-18808-Ljbffr ABL Health Care, LLC
