Speech Language Pathologist

Role Overview Seeking a licensed Speech Language Pathologist to provide personalized, one-on-one care to older adults in their homes or senior living communities. The role involves conducting evaluations, developing tailored treatment plans, and delivering evidence-based therapy to enhance mobility, independence, and quality of life. Key Responsibilities Perform assessments and create individualized care plans Deliver therapy sessions supporting patient wellness and functional independence Document care in EMR systems per regulatory standards Collaborate with interdisciplinary teams including PTs, OTs, and fitness specialists Educate patients, caregivers, and staff to promote safety and aging in place Incorporate telehealth and virtual care to increase flexibility and access Qualifications & Skills Valid state licensure and CCC from ASHA Degree from an accredited Speech Language Pathology program Proficiency in basic computer and EMR use Current CPR certification Ability to meet physical demands including lifting, repositioning, and driving Benefits & Environment Join a clinician-driven, collaborative practice supporting independence, wellness, and professional growth.
