Key Responsibilities\n

As a Licensed Master Social Worker with Bright Path Social Work, your responsibilities will include:

• Advocating for clients to access community resources, such as healthcare, financial assistance, and social services, to enhance their quality of life.

• Regularly monitoring clients through home visits and virtual check-ins to assess their living conditions and provide necessary interventions.

• Facilitating clear and effective communication between family members regarding care strategies, concerns, and updates.

• Educating clients and families about available local community resources and support systems to foster social engagement.

• Maintaining meticulous records of client interactions, assessments, care plans, and progress notes, ensuring compliance with ethical and legal standards.

• Staying updated on current best practices in social work and attending relevant training to continuously improve service delivery.

Patient Population / Client Focus\n

Your primary focus will be working with senior clients who face various challenges related to aging, including mental health issues, chronic health conditions, and social isolation. You will be pivotal in fostering their emotional and psychological resilience, while also addressing practical concerns related to their care and daily living.

Qualifications\n

• A Master’s degree in Social Work from a program accredited by the Council on Social Work Education.

• Current and valid licensure as a Licensed Master Social Worker (LMSW) in the state of Florida.

• A minimum of two years of relevant experience in social work, particularly in a geriatric or community-focused setting.

• Strong understanding of social work principles, challenges related to aging, and knowledge of community resources.
