Peer Specialist

Liberty Resources is seeking an Adult Peer Support Specialist to work in our Integrated Health Care Clinic in Fulton, NY. The Adult Peer Support Specialist will provide supportive services with a rehabilitation and recovery focus, designed to promote coping skills for behavioral health symptoms while facilitating the utilization of natural resources and enhancing recovery-oriented principles. Responsibilities Provide one-to-one supportive services to individuals on‑site and in the community in accordance with the APS Code of Ethics and/or NYCB Code of Ethical Conduct. Support clients during Same Day Access (SDA) hours, reviewing needs and PRAPARE screening responses, identifying resources, and offering peer‑based support aligned with the SDA workflow. Conduct outreach and community‑based visits to support engagement, promote access to care, and reduce service barriers. Coordinate and collaborate with care managers, patient navigators, and clinical teams to ensure a seamless service experience. Act as a positive role model, demonstrating personal wellness and recovery. Complete documentation such as assessments, progress notes, and care plans per program and agency standards. Assist individuals in identifying and utilizing personal recovery tools, relapse prevention plans, wellness activities, and self‑help resources. Provide recovery education and coaching, emphasizing community involvement, wellness, spirituality, employment, and empowerment. Use a personal vehicle for community‑based appointments as needed. Respond flexibly to emergency or unplanned situations, which may involve schedule or location changes. Value service, excellence, and responsibility, seeing opportunities where others see barriers. Perform other duties as assigned. Qualifications High School Diploma or GED. Personal experience as a recipient of mental health and/or substance abuse services, with willingness to share experience to benefit clients. Demonstrated ability to maintain personal wellness and utilize recovery tools. Certified by the Office of Mental Health (OMH) as a Certified Peer Specialist or Office of Alcoholism and Substance Abuse (OASAS) certified Peer Recovery Advocate, or willingness to become certified within 6 months of hire. Ability to work collaboratively with a multidisciplinary team. Ability to effectively use an electronic health record. Excellent communication, organizational, engagement, and interpersonal skills. Valid NYS driver's license and reliable transportation. Salary Salary Range: $50,000 to $55,000. Benefits Collaborative and supportive team atmosphere.
