JOB SUMMARY:

An in-home care Registered Nurse (RN) will help assess and formulate individualized care plans for clients while directing care staff on day to day needs to maximize quality life.

RESPONSIBILITIES:

Manage a caseload of clients assigned by supervisor
Provide oversite to client care to clients with varying backgrounds and medical histories
Oversee safe medication administration practices
Complete required documentation timely and accurately
Assess clients for change ofcondition
Keep clients family andphysician informed of any changes in client behavior, medications, orconditions of health
Assign and provide instruction for home health aides
Communicate needs of residentto family and outside providers
Comply with HIPPA regulations in an out of the office
Other Duties as Assigned

REQUIREMENTS:

Registered Nurse (RN) Licensure in the State of practice
