Develops and implements Care Plans through Chronic Care Management Services Coordinates continuity of patient care with patients and families following hospital admission, discharge, and ER visits Educates patient and family on ways to reduce ER visits in the future Participates in Patient-Centered Medical Home team meetings, quality improvement initiatives and Value Based Care Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
