We are seeking a Full-time Licensed Practical Nurse (LPN) to join our skilled nursing team at Vivo Healthcare Wauchula in Wauchula, Florida. As an LPN, you will play a vital role in delivering high-quality patient care, working closely with our medical team to ensure the best possible outcomes for our residents. Responsibilities:

• Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians

• Develop and implement individualized care plans, taking into account each resident's unique needs and goals

• Collaborate with the interdisciplinary team to identify and address concerns, and implement changes to improve care

• Monitor and report patient status, attending to changes in condition and alerting staff as needed

• Maintain accurate and up-to-date records, including documentation of patient care and medications administered
