OASIS Workflow Coordinator

Hartford Healthcare seeks an OASIS Workflow Coordinator to oversee and optimize OASIS documentation across our home health services. In this role, you will coordinate intake and clinical teams, review OASIS submissions for accuracy and compliance, track metrics, and implement process improvements that support high-quality patient care. You will train staff on OASIS standards, collaborate with clinical leadership, and leverage EMR systems to ensure timely, complete, and compliant documentation in a culture of teamwork, compassion, and excellence.
