Home Health Certified Nursing Aide CNA

Job Summary:

Pathwell Health, a trusted and community-focused home health care provider, is seeking a compassionate and skilled Home Health Certified Nursing Aide (CNA) to join our team. As a Home Health CNA, you will provide routine individualized healthcare to elderly, convalescents, or persons with disabilities at their homes or in care facilities. Essential Duties and Responsibilities:

• Provide companionship and socialization to clients

• Escort clients to recreational activities and medical appointments

• Perform light housekeeping duties to maintain a safe environment

• Help clients with personal care needs, such as medication reminders and Activities of Daily Living (ADLs)

• Assist clients with self-care skills and maintain complete and accurate documentation of care

• Maintain confidentiality in all matters related to client care

• Perform other duties as assigned

Minimum Qualifications:

• One year of caregiving experience (preferred)

• Part-Time role available

• West Virginia HHA (preferred) or CNA (required), Driver's License, Car Insurance, and Car Registration

• Strong clinical assessment, organizational, and communication skills

• Ability to handle multiple assignments simultaneously

• Excellent oral and written communication skills

• Excellent customer relationship skills

The noise level in the work environment is usually low to moderate.
