Smaller community healthcare facility part of a system looking to bring on Case Management Director! Sign On Bonus and Full Relocation! Responsibilities Direct reports include 4 full-time staff and numerous prn staff Team consist of all Social Workers Typical case load expected of the Case Managers is 11-14 patients Director will carry a caseload as needed but should expect weekly to have front line responsibilities. Qualifications Degree required, must possess bachelor’s degree in Nursing (BSN) with RN licensure Degreed required in SW, OT, PT, ST, or Rehabilitation Counseling. 3 years of hospital-based Case Management experience including Utilization Review and Discharge Planning #J-18808-Ljbffr
