Job Title: Physician Assistant / Nurse Practitioner - Post-Acute Care

Required Skills:

• NP/PA certification required
• Active NY license or ability to obtain
• At least one year of post-acute care experience preferred but not required
• Excellent communication and teamwork skills
• Ability to work independently in a collaborative setting

Preferred Education and Experience:

• Bachelor's degree or higher in Nursing or Physician Assistant studies
• Prior experience in SNF or rehabilitation settings is a plus

Other Requirements:

• Full-time commitment, Monday through Friday (5 days/week)
• Additional certifications or licenses as required
• No travel required
