Senior Home-Visit Primary Care Clinician
SCAN Group
SCAN Group is seeking an Advanced Practice Clinician to provide and coordinate primary care for seniors in Maricopa County, including Mesa, Gilbert, Tempe, and Chandler. You will conduct exams, vaccines, and care planning in homes and facilities, with ongoing collaboration across care teams. Responsibilities include triage, documentation, and annual reviews to close gaps in care. A NP/PA license in AZ is required with travel up to 75% across the assigned area. #J-18808-Ljbffr SCAN Group
