Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Reassess cost-efficient, appropriate levels of care
• Coordinate non-medical support to ensure compliance with treatment plans
• Coordinate care transitions with physicians, social workers, and claims professionals
• Engage specialty resources as needed
• Keep detailed records of clinical and fiscal outcomes
• Identify opportunities for health promotion and illness prevention
• Act as a patient advocate, protecting privacy and confidentiality

Requirements of the RN - Case Management

• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted Arkansas and Compact RN license
• Case Management certification
• Proficient with HCHB and Home Health Oasis
• 1 year of Home Health experience
• Ability to analyze complex medical information and make rational decisions
