Role Description This is a full-time, on-site Nurse Practitioner role based in Modesto, CA. The Nurse Practitioner will conduct comprehensive patient assessments, obtain medical histories, perform physical examinations, and order and interpret diagnostic tests related to cardiovascular conditions. Responsibilities include developing and managing treatment plans, prescribing medications within scope of practice, and providing patient education on disease management, medications, lifestyle changes, and preventive care. The role involves collaborating closely with cardiologists, nurses, and other healthcare professionals to coordinate inpatient and outpatient care, including follow-up visits and chronic disease management. The Nurse Practitioner will also document all encounters in the electronic health record, support quality and safety initiatives, and participate in practice meetings and case reviews as needed. Qualifications Current, unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) license in the state of California, with national board certification as an NP (e.g., AANP or ANCC). Graduation from an accredited Nurse Practitioner program; master’s or doctoral degree in Nursing or related field preferred. Clinical experience in cardiology, internal medicine, or a related acute or primary care setting strongly preferred. Demonstrated ability to perform focused cardiovascular assessments, interpret common cardiac diagnostics (e.g., EKGs, echocardiograms, stress tests), and manage chronic cardiac conditions under physician collaboration. Strong skills in patient counseling, education, and shared decision-making, with an emphasis on culturally sensitive and inclusive communication. Proficiency with electronic health records, clinical documentation, and basic office technology. Ability to work collaboratively within a multidisciplinary team, manage a steady patient volume, and prioritize tasks in a fast-paced clinical environment. Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required or obtained within a specified time frame after hire. Commitment to professional ethics, patient confidentiality, and adherence to evidence-based guidelines and practice policies.
