Registered Nurse (RN) – Home Health | Northern West Virginia

We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care throughout Hancock, Brooke, Ohio, Marshall, Wetzel, and Tyler Counties, West Virginia .

Schedule & Job Details

• Schedule: Monday–Friday, 8:00 AM – 5:00 PM
• Hours: 40 hours not guaranteed
• On-Call: Two weekends (Saturday & Sunday) per month
• Territory: Hancock, Brooke, Ohio, Marshall, Wetzel, and Tyler Counties, WV
• Mileage Reimbursement: $0.49 per mile
• Productivity Requirement: 30 points per week

Requirements

• Active West Virginia RN License or Compact RN License
• Previous Home Health experience preferred
• Proficiency with OASIS documentation
• Experience using HomeCare HomeBase (HCHB)
• Strong medical-surgical (Med Surg) nursing skills
• Experience with:
• • IV therapy
  • Lab specimen collection (lab sticks)
  • Wound VAC management
• Excellent assessment, communication, and case management skills

Responsibilities

As a Home Health Registered Nurse, you will provide comprehensive nursing care, coordinate patient services, and develop individualized care plans that promote health, independence, and improved quality of life.

Key responsibilities include:

• Perform comprehensive patient assessments and ongoing clinical evaluations.
• Develop, implement, and update individualized care plans based on physician orders and patient needs.
• Provide skilled nursing care for patients with acute, chronic, and complex medical conditions.
• Coordinate care with physicians, therapists, case managers, and other members of the interdisciplinary healthcare team.
• Monitor patient progress and promptly communicate changes in condition to the physician.
• Manage medications, including ordering, monitoring, documentation, and patient education.
• Ensure compliance with medication administration, storage, safety, and documentation policies.
• Complete accurate and timely clinical documentation, including OASIS assessments and HCHB charting.
• Maintain required nursing documentation, including daily, monthly, quarterly, and annual reports.
• Perform skilled nursing procedures, including IV therapy, wound VAC management, laboratory specimen collection, and other Med Surg-related interventions.
• Supervise Licensed Practical Nurses (LPNs) and other clinical staff as appropriate.
• Coordinate physician appointments, specialty referrals, and follow-up care.
• Collaborate with hospital discharge planners and inpatient nursing teams to ensure seamless transitions of care.
• Provide patient and caregiver education focused on disease management, medication adherence, and health maintenance.
• Train new employees and provide annual health and safety education to clinical staff.
• Ensure infection prevention standards are maintained for equipment, supplies, and patient care.
