Children's Care Coordinator

New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24‑hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. Additionally, we serve 22,000 students in area schools and collaborate with courts, law enforcement, jails, and hospitals to improve the health of individuals and quality of life in our community. We are seeking a caring and compassionate Care Coordinator who is eager to teach, deliver meaningful work, and believes that his or her efforts can make a difference in the lives of the people we serve. Responsibilities Provides time‑limited care coordination for 12‑15 identified individuals using a combination of internal utilization reviews and SEFBHN high‑utilizer recommendations. Coordinates services until the individual is adequately connected to the care that meets their needs and progress toward the goals of Care Coordination are achieved. Ensures individuals are engaged in their current setting (e.g., crisis stabilization unit (CSU), SMHTF, homeless shelter, detoxification unit, addiction receiving facility, etc.) to establish a warm hand‑off. Guarantees that an internal process is in place that allows for on‑call services to be available 24 hours, seven days a week. Models, coaches, and supports shared decision‑making in care planning and service determination with the individual and family members (where applicable) and emphasizes self‑management, recovery and wellness, including transition to community‑based services and/or supports. Works with SEFBHN to develop diversion strategies to prevent individuals who can be effectively treated in the community from entering SMHTFs. Requirements Master’s degree from an accredited university or college with a major in counseling, social work, psychology, or a related human services field and two years full‑time or equivalent experience working with adults and children at risk. Professional license highly preferred. Must pass Level 2 Background through DCF and AHCA Clearinghouse.
