Be the clinical leader of Headquarters.

We're building the virtual clinic that should already exist for every concussion patient in America: clear diagnosis, evidence-based recovery, interdisciplinary specialists, and AI-enabled follow-through. The Medical Director is our top clinical seat today. You will define how care is delivered, hire and elevate the clinicians who deliver it, represent us in the medical world, and see patients as our leading concussion specialist. As we scale, this role is designed to become Chief Medical Officer, with expected relocation to San Francisco for in-person leadership as you transition full-time.

About the Role:

• This is the founding clinical leadership role at Headquarters Health: Medical Director now, with a clear path to Chief Medical Officer as the company and clinic scale.
• You will be the medical authority our founders partner with daily: owning clinical strategy, protocols, quality, provider hiring and oversight, and external medical credibility.
• You will also practice. The best clinical leaders here stay close to patients, complex cases, and the real friction of telehealth concussion care.
• Start remote and part-time/contract. As the role grows into full-time CMO-level leadership, we expect relocation to San Francisco for in-person collaboration with the founding team.

What You'll Do:

• Own the clinical vision for our virtual concussion and mTBI clinic, from first evaluation through return to school, sport, work, and life
• Define, approve, and continuously improve evidence-based protocols, standing orders, escalation paths, and interdisciplinary care pathways
• Build and lead the clinical team: recruit, interview, credential, onboard, mentor, and manage physicians, NPs/PMHNPs, PTs, therapists, and other specialists
• Serve as collaborating / supervising physician where state law requires it, with real chart review, feedback, and quality standards (not rubber stamps)
• Practice as our senior concussion specialist via telehealth: complex cases, high-stakes decisions, and care that sets the bar for the whole team
• Own clinical quality, safety, documentation standards, and multi-state telehealth compliance with clinical ops
• Partner with product and engineering so AI tools, patient companion workflows, and care pathways reflect best-in-class concussion medicine
• Lead medical conversations with schools, athletic organizations, health systems, specialists, and partners
• Help shape clinical research, outcomes measurement, and the story of why Headquarters care is different
• Advise founders on clinical hiring, medical risk, go‑to‑market with clinicians, and when to open new states or service lines
• [Near-term growth] Formalize into Chief Medical Officer as we scale headcount, states, and clinical volume
• [Future] Build and mentor a national network of concussion clinicians under one medical standard

Qualifications:

• MD or DO with active, unrestricted U.S. medical license (multi‑state licensure strongly preferred; we will fund additional licenses)
• Board certification in a relevant specialty (sports medicine, neurology, family medicine, pediatrics, physiatry, emergency medicine, or equivalent concussion‑focused practice)
• Deep clinical experience with concussion / mTBI: ideally 5+ years, including adolescents and/or athletes
• Proven clinical leadership: Medical Director, collaborating physician, service‑line lead, fellowship director, or equivalent ownership of protocols and people
• Telehealth experience and comfort governing remote, multi‑state care
• Fluency with current concussion consensus guidelines and progressive, multimodal rehab (not rest‑only dogma)
• Willingness and ability to supervise / collaborate with APPs and interdisciplinary clinicians under applicable state laws
• Exceptional communication with patients, families, athletic trainers, schools, and specialist peers
• Clean disciplinary history; able to complete credentialing and NPDB review
• Open to relocating to San Francisco as the role transitions to full-time, in‑person leadership

Bonus:

• Already in the Bay Area / San Francisco, or actively excited to relocate
• Recognized concussion or sports medicine reputation (publications, speaking, team/school affiliation, or referral network)
• Prior startup, digital health, or multi‑state virtual clinic leadership
• DEA registration and comfort with telehealth prescribing where clinically appropriate
• Experience building clinical hiring pipelines and training programs
• Comfort advising product/engineering teams and translating medicine into software workflows
• Existing multi‑state licenses or IMLC pathway experience

Schedule:

• Early phase: flexible remote schedule with reliable weekly ownership of clinical decisions and team availability
• Patient visits, when scheduled, are booked 24+ hours in advance

Compensation & Benefits:

• Pay: $150-$250 per hour + significant equity to start; compensation evolves with full‑time SF transition (Expected hours: 5-15 per week initially, with room to grow)
• Work Location: Remote (U.S.) to start; expected relocation to San Francisco for in‑person, full‑time leadership
