Position: Outreach Coordinator


The Community Outreach & Care Continuity Specialist bridges the gap between public health initiatives and target populations. This role focuses on active community engagement, connecting at-risk or disengaged individuals to healthcare services, and guiding them toward long-term care stability and retention.The Community Outreach & Care Continuity Specialist bridges the gap between public health initiatives and target populations. Schedule: Mon-Fri 8:30am-5:00pm


Location: Claremont, CA


Must have a Bachelor Degree


Must have a valid driver license, automobile and auto insurance


Pay: $26.50-28.50/hr
