Conduct comprehensive nursing assessments, identify barriers to care, and develop patient-centered care plans in collaboration with the interdisciplinary team. Coordinate services across the continuum, including referrals, follow‑up plans, and appropriate disposition decisions. Educate patients and families on conditions, treatment options, medications, and self‑management strategies. Document care plans, interventions, and outcomes accurately in the electronic health record; monitor progress and adjust plans as needed. Communicate effectively with physicians, social workers, and other care team members to ensure timely, safe, and appropriate patient care. Requirements Ability to work in an outpatient urgent care setting. Strong clinical assessment, critical thinking, and communication skills. Proficiency with electronic health records and documentation. Willingness to participate in a travel assignment with a fixed 12-week duration and 40 hours per week. Demonstrated ability to collaborate with a multidisciplinary team and coordinate patient care transitions.
