Registered Nurse Case Manager

The Registered Nurse - Case Manager is responsible for coordinating and managing patient care within an acute care facility. This role involves overseeing care coordination and discharge planning to ensure optimal patient outcomes. The Nurse will collaborate with interdisciplinary teams, manage patient caseloads, and maintain compliance with clinical standards. A minimum of two years of acute medical care management experience is required, along with an active California RN license. Responsibilities Coordinate and manage patient care and discharge planning in an acute care setting. Assess patient needs and develop individualized care plans. Collaborate with interdisciplinary teams to ensure continuity of care. Maintain accurate documentation and compliance with facility policies. Manage patient caseloads with typical ratios of 1:25 during day shifts. Communicate effectively with patients, families, and healthcare providers. Participate in care coordination and case management activities. Required Skills & Certifications Active Registered Nurse license in the state of California (pending license accepted if active by start date). Minimum of 2 years acute medical care management experience in care coordination or discharge planning. Strong clinical assessment and patient management skills. Excellent communication and interpersonal skills. Ability to work in a fast-paced acute care environment. Preferred Skills & Certifications Experience with case management in acute care facilities. Knowledge of care coordination best practices and discharge planning processes. Familiarity with electronic medical records and documentation systems. Special Considerations Weekend work typically required one full weekend per four weeks, subject to department needs. Floating to other units is not required. First-time travel nurses accepted but must undergo screening. Time off restrictions apply; no more than 7 days of requested time off allowed. Dress code requires business attire or scrubs. Scheduling Day shift with patient ratios approximately 1:25. Schedule includes typically one weekend per month. Shift assignments based on department needs and staffing requirements. #J-18808-Ljbffr
