Case Manager Location: Westwood, MA Shift: Monday-Thursday 8:00am-4:00pm, Friday 7:30am-3:30pm Salary: $50,473 per year Responsibilities Provide individual and small group instruction in self‑care, activities of daily living, communication, social and motor skills. Provide direct care for activities of daily living and group activities; lift and transfer individuals as needed. Coordinate services with the interdisciplinary team. Communicate respectfully and proactively with families, residential staff, and all members of participants’ teams. Develop and implement Day Habilitation Service Plans (DHSP), maintaining progress notes, participant records, bi‑annual and annual summaries. Complete required documentation in a timely and accurate manner. Participate in scheduled supervision sessions, program‑wide staff meetings, case review meetings, and team meetings on program development, curriculum, and schedules. Demonstrate good computer skills and ability to learn agency computer programs for documentation. Attend required in‑service trainings, multitask and prioritize tasks, and work effectively in a team environment. Maintain strong written and verbal communication and excellent interpersonal skills. Comply with all applicable state and federal regulations, agency policies and procedures, and personnel practices. Protect the health and safety of all participants and staff. Perform other duties as requested by supervisor(s). Qualifications Experience working with individuals with intellectual and developmental disabilities. Bachelor’s or associate degree in a related field with at least two years of experience, or high‑school diploma with five years of related human services experience. Ability to pass required background checks including fingerprint, CORI, DPPC, driving, and a valid driver’s license. MAP certification preferred. Excellent verbal and written communication skills.
