Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join as a traveling Physical Therapist providing home health services in Cadillac, MI and surrounding communities. You will help patients recover from injuries and manage chronic conditions while working within an interdisciplinary home health team. Minimum Qualifications

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program.
• Active Michigan physical therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience with Home Care Home Base EMR preferred.
• Productivity expectation: 30 points per week.
• Ability to complete documentation and visit plans in a home health setting.
• Reliable transportation and ability to travel within an approximately one-hour radius of the hub city based on referral distribution.

Preferred Qualifications

• Previous home health physical therapy experience preferred but candidates new to home health will be considered.
• Experience conducting evaluations, discharge planning, and working with interdisciplinary home health teams.
• Comfort using video conferencing for interviews (Teams/Video Call).

Assignment Details

• Shift days: Monday through Friday.
• EMR: Home Care Home Base.
• On call: None.
• Mileage reimbursement: IRS standard mileage rate.
• Expect to cover patients within roughly a one-hour driving radius of the hub city, with exact coverage determined by referral locations.
