Nurse Practitioner

Nurse Practitioner - PART_TIME Carease Health - OH is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner with our team, you will have the opportunity to work in a hybrid environment, combining on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Job Responsibilities: Provide high-quality preventative care to residents in skilled nursing facilities Conduct on-site visits and remote work from home Collaborate with facility staff to develop and implement patient care plans Utilize Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to improve patient outcomes Maintain accurate and timely documentation using a dedicated scribe team Build strong relationships with facility staff and patients Participate in quality improvement initiatives to enhance patient care standards Requirements: Active NP or PA license in the state of Ohio 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills Ability to work independently and as part of a team Strong patient-centered approach Additional Benefits: Hybrid role with flexible scheduling Support and training from our clinical directors, regional managers, and supervisors Generous Paid Time off Health Insurance for full-time employees Free CEUs Referral Bonuses #J-18808-Ljbffr Careasehealth
