Tryfacta is seeking an LPN - LTC for our client in Berlin, NH 03570. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please apply now!

Location: Berlin, NH 03570

Duration: 13 weeks (possible extension)

Work Schedule: 8/16 hrs. (Day, Evening and Night) Guaranteed Hours: 40/wk.

Pay rate: Local - $45/hr. and Traveler: $50/hr. ($2000/wk.) Overtime: Yes (including potentially one extra shift.) Total Beds: 100 Current Census: 96 Shift: Primarily 2nd and 3rd shifts

Responsibilities for this position include, but are not limited to:

• Provide direct nursing care to residents in a long-term care setting.
• Administer medications and treatments according to physician orders and facility protocols.
• Manage approximately 15 patients per medication pass, with Medication Aides assisting with certain medications.
• Assess residents and monitor for changes in physical, emotional, and clinical condition.
• Report significant changes in resident condition to the appropriate healthcare provider.
• Provide patient education regarding medications, treatments, health conditions, and care plans.
• Provide guidance, education, and support to LNAs/Nurse Aides.
• Collaborate with Medication Aides, Nurse Aides, RNs, physicians, and other members of the healthcare team.
• Assist with development and implementation of individualized care plans.
• Maintain accurate and timely patient documentation using PointClickCare (PCC).
• Follow infection-control, safety, and facility policies.
• Promote resident dignity, comfort, safety, and independence.
• Support quality patient care while maintaining a strong bedside presence.
• Work effectively within established staffing ratios and assist with unit needs as required.

To be considered for this position, you should have:

• Active LPN/LVN license.
• LTC/SNF experience preferred.
• Medication administration experience.
• PCC experience preferred.
• Strong communication and patient-care skills.
• Available for 40 hours/week, primarily 2nd/3rd shifts.
• Willing to work 8- and 16-hour shifts.
