Registered Nurse Case Manager

Del Sol Medical Center in El Paso seeks a Registered Nurse Case Manager to coordinate safe, patient-centered care across the continuum in a busy acute care setting. In this role, you’ll assess patient needs, develop and implement care and discharge plans, and collaborate closely with physicians, nurses, social workers, and community providers. You’ll perform utilization review, ensure regulatory and payer requirements are met, and connect patients with appropriate post-acute services.
