Job Summary Per Diem Clinician (Nurse Practitioner or Physician Assistant) Company: Signify Health, part of CVS Health Location: Providence, RI (able to cover primary locations: Providence, RI; may travel locally and within licensed states) Job Type: Part time Anticipated Weekly Hours: 12 Responsibilities Bring your heart into every visit, joining a national network of purpose-driven clinicians dedicated to improving health outcomes. Be part of our efforts to visit millions of people nationwide to provide connections to the right care. Conduct visits in patients' homes, performing medication and medical history reviews, physical evaluations, and diagnostic tests (e.g., diabetic eye exams, spirometry). Use an intuitive clinical workflow and an iPad for each evaluation. Conduct virtual visits as needed. Provide all Signify Health services as indicated, including diagnostic and preventive services (spirometry, DEE, bone density screening, etc.). Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi-state licenses preferred. Ability to cover the primary location: Providence, RI. Compensation & Benefits Hourly base rate, plus additional incentive structure (per visit completed, other incentives may apply). Malpractice insurance coverage during Signify Health-related activities. Supplies and other perks. Pay Range: $0.00 - $99.14 per hour (actual base rate depends on experience, education, geography and other factors).
