Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee. Although based out of Nashville, we serve several communities across middle and East Tennessee with satellite offices in Antioch, Gallatin, Dickson, Columbia, Cleveland, Murfreesboro, Clarksville, Cookeville, Chattanooga, and Memphis. Tuesday- Saturday 10AM-6:30pm Assist MHC staff with transportation for their consumers to essential appointments concerning their treatment and recovery. Must have F Endorsement. Position Responsibilities: Transport consumers to and from designated appointments. Transport medications to be delivered to consumers. Communicate with the team regarding consumer concerns and/or transportation issues for the consumer. Attend team, department and Agency meetings as required. Maintain the MHC vehicles in optimal condition and report any concerns to the Transportation Supervisor. Must inspect assigned vehicle at the beginning of work shift and complete vehicle inspection form. Complete all required paperwork in a legible and timely manner. Respond expediently to all communication with office. Responsible for managing route, which includes calling consumers prior to leaving the office and planning route effectively. Other Responsibilities: Responsible for attending all mandatory in‑services, continuing education and annual health requirements. Comply with all Corporate Compliance policies, practices and procedures Report all unethical activities to supervisor, Corporate Compliance Office, Values Hotline number or Human Resource Director. Comply with all OSHA and safety policies, practices, and procedures Report all unsafe practices or accidents to supervisor, Safety Supervisor and/or OSHA Coordinator. Will be compliant with the relevant regulations concerning the privacy and security of consumer’s protected health information (PHI) as established by the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Participate in proactive team efforts to achieve departmental and company goals. Perform other duties as assigned. Provide leadership to others through example and sharing of knowledge/skill. Follow all policies and procedures set by the Agency. MHC was founded in Nashville, Tennessee, in response to the need for assertive community‑based treatment for individuals who have required frequent inpatient psychiatric hospitalization. Since 1993, MHC has been passionately helping adults with severe mental illness to live quality lives in the community with our state‑of‑the‑art treatment and support. In 1999, MHC recognized the need to support the whole family and expanded services to include children and youth. We established our non‑profit agency by merging three community organizations: Urban Case Management of Meharry Community Health Center, Continuous Treatment Team of Council of Community Services, and Mobile Crisis Team of the Crisis Intervention Center. This fusion resulted in an innovative, comprehensive, and multidisciplinary approach that addresses the specialized needs of individuals living with severe behavioral health conditions and challenges related to poverty.
