Physical Therapist – Home Health

Schedule - Shift: 8-hour days only - Weekends: Not required

Assignment Details - Guaranteed Hours: Facility may call off up to one shift per pay period

Requirements - License: Pending license accepted - Certifications: BCLS required - Experience: 1 year of skilled home health experience required - Unit-specific skills: Home health physical therapy skills including orthopedic post-operative care, neuro rehabilitation, gait and mobility training, home exercise programs, patient and family education, safe body mechanics

Roles & Responsibilities

Provide patient assessments and develop appropriate treatment plans in accordance with industry standards.

Implement and supervise therapeutic interventions that aim to improve patients' physical functioning.

Educate patients and families regarding treatments and techniques for home exercises and physical rehabilitation.

Document patient progress and outcomes in compliance with facility standards.

Collaborate with interdisciplinary teams to optimize patient care and coordinate rehabilitation strategies.

Charting systems: Experience with OASIS required; familiarity with electronic charting systems is expected.

Orientation/modules: Pre-start modules required.

Transportation: Clinician expected to provide own reliable transportation.

Vaccine: Confirm COVID-19 vaccine requirement with hiring manager before submission.
