Home Health Physical Therapy Assistant (PTA)

Location: West Portland, OR and surrounding areas

Assignment Duration: This is a 13-week contract position.

Schedule: Day shift, 5x8-hour schedule (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Job Description: We are seeking a dedicated and skilled Home Health Physical Therapy Assistant (PTA) to provide individualized care to patients in their homes. The ideal candidate will have experience in skilled home health settings, a commitment to quality patient care, and the ability to manage multiple patient visits effectively.

Responsibilities

• Deliver high-quality physical therapy services to patients in their homes.
• Implement treatment plans designed by a licensed Physical Therapist.
• Assist patients with mobility and physical rehabilitation exercises.
• Monitor and document patient progress and report to the supervising Physical Therapist.
• Maintain effective communication with patients, families, and healthcare team members.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience is required.
• BCLS certification is required.
• Candidate must have reliable transportation and be comfortable driving up to 60 miles per day for patient visits.
• Primary series COVID vaccination plus one booster is required.
