ENDO Nurse Practitioner (Nursing Home)

We are seeking a compassionate and dedicated Endocrinology Nurse Practitioner (NP) to join our dynamic team. In this role, you’ll play a vital part in delivering comprehensive endocrine care to patients, working closely with an interdisciplinary team in both long-term care and acute settings. Responsibilities Provide on-site endocrinology consults in nursing home settings, including initial assessments for new admissions and complex endocrine conditions such as diabetes, thyroid disorders, osteoporosis, and adrenal disorders, as well as routine weekly follow-up visits and care adjustments based on findings. Conduct comprehensive evaluations by gathering medical histories, performing endocrine-focused physical exams, and ordering and interpreting labs such as A1C, thyroid panels, metabolic panels, and bone density tests. Collaborate with endocrinologists, interdisciplinary teams, nursing staff, and facility providers to develop treatment plans, manage complex metabolic conditions, and reduce hospital readmissions. Administer and manage medications, including insulin and endocrine therapies, monitoring treatment responses and adjusting regimens appropriately. Educate patients, residents, and families on diabetes management, nutrition, hormone disorders, and medication adherence. Provide emotional support and counseling to patients and caregivers. Ensure accurate documentation in compliance with OSHA and governmental regulations, including thorough EMR entries and transition summaries to ensure continuity of care across settings. Maintain familiarity with CMS regulations and NCQA HEDIS guidelines. Participate in quality improvement programs aimed at improving chronic disease management in skilled nursing facilities. Qualifications Minimum of 6 months of experience in a long-term care or skilled nursing facility (hospital or outpatient office experience also considered). Active and unrestricted NP license. DEA license required. CPR certification (BCLS or BLS). Master’s degree in Nursing (NP applicants). Graduation from an accredited NP program. Preferred experience in endocrinology (training available if not). Preferred familiarity with EMR systems such as Sigma, Visual, or PCC (training provided). Strong organizational, communication, and time‑management skills.
