Physical Therapist Case Manager

CarePartners seeks a Physical Therapist Case Manager to support members in the Franklin area through coordinated, patient-centered home-based care. In this role, you will manage a caseload of health plan members, perform functional assessments, develop and implement care plans, and collaborate closely with physicians, nurses, and community providers.
