Job Summary

We are seeking an experienced Registered Nurse Case Manager to join a leading acute care healthcare team. This full-time permanent opportunity offers competitive pay, excellent benefits, and career growth potential.

Schedule

• Full-Time
• Day Shift: 8:00 AM – 4:30 PM
• Rotating Weekends

Compensation & Benefits

• Pay Range: $34.25 – $47.10/hr

Responsibilities

• Coordinate patient care and discharge planning
• Conduct patient assessments and develop individualized care plans
• Collaborate with physicians, nurses, and interdisciplinary teams
• Educate patients and families regarding treatment plans and available resources
• Evaluate discharge needs and facilitate smooth transitions of care
• Ensure appropriate utilization of healthcare resources
• Advocate for patient safety and quality outcomes

Requirements

• Active RN License
• Associate Degree in Nursing or Nursing Diploma
• Minimum 2 years of recent Acute Care experience within the last 3 years
• Experience in one or more of the following:
• • ICU
  • CCU
  • Med-Surg
  • LTAC
  • Case Management
