Lakeland Nursing and Rehab is seeking compassionate and skilled  Registered Nurses (RNs) to join our dedicated care team. If you’re passionate about providing exceptional resident care while growing as a leader, we’d love to meet you.

 

Key Responsibilities:

· Provide direct patient care to residents, including assessing, planning, implementing, and evaluating care plans

· Administer medications, perform wound care, and conduct vital signs as needed

· Collaborate with other members of the healthcare team to ensure comprehensive care

· Develop and implement educational programs for residents and families

· Communicate effectively with residents, families, and healthcare team members

· Maintain accurate and detailed records of patient care and treatments

· Participate in quality assurance and improvement activities to ensure high-quality care

• Foster a positive, team-oriented, and collaborative work culture

• Contribute to quality assurance initiatives, budget planning, and internal committees

• Ensure accurate MDS documentation and coordinate discharge planning with families

• Provide individualized, compassionate care to residents and their loved ones

 

Qualifications:

• Active RN license in the State of New Jersey

• Minimum 1 - 2 years of SNF (Skilled Nursing Facility) experience

• Excellent communication, organization, and critical thinking abilities
