Licensed Practical Nurse (LPN)
We're seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Greenfield Skilled Nursing and Rehabilitation. Responsibilities
- Deliver compassionate, safe, and effective nursing care based on individualized care plans
- Administer medications and treatments as prescribed by the physician
- Monitor patient/resident conditions and report changes to the supervising nurse or physician
- Document all care and observations accurately and timely in accordance with regulations
- Assist with wound care, catheter care, tube feedings, and other clinical procedures
- Supervise nursing assistants and ensure care standards are upheld
- Participate in care planning, team meetings, and shift reports
- Support residents' dignity and well-being through person-centered care
Requirements
- Current, valid Licensed Practical Nurse (LPN) license in the State of Ohio
- Current valid hands-on CPR Certification
- Strong communication and teamwork skills
- Passion for senior care and a commitment to excellence
