RPh on the Go has partnered with a pharmacy in Texas seeking an Infusion Pharmacist for a 13-week contract assignment.
Position Details
- 13-week contract
- 12:00 PM–8:00 PM shift
- Home infusion pharmacy setting
Key Duties
- Assess patients for home-based infusion therapy
- Review medication orders, labs, dosing, and therapy appropriateness
- Compound and verify sterile medications following USP requirements
- Monitor patient response and make therapy recommendations when appropriate
- Provide medication and infusion education to patients and caregivers
- Coordinate with physicians, nurses, and other healthcare professionals
- Oversee infusion pumps and related administration equipment
- Support quality assurance, documentation, and medication safety initiatives
Qualifications
- Active Texas Pharmacist license
- PharmD or graduate of an accredited pharmacy program
- Previous sterile IV, compounding, clinical pharmacy, residency, APPE, or equivalent experience
- Ability to interpret laboratory values and utilize pharmacokinetic principles
- Strong communication and patient counseling skills
