Field Case Manager - Housing & Support (Bilingual)

Bond Health is seeking a full-time Case Manager in New York, NY, responsible for providing residential and long-term support services to homeless and at-risk adults. You'll carry a caseload of 20 to 25 individuals, assist with obtaining resources, and facilitate groups focused on life skills. A Bachelor's degree in human services and bilingual Spanish are required.
