Responsibilities
- Conduct initial and ongoing physical therapy assessments for residents in a long-term care setting; develop and implement individualized treatment plans.
- Perform therapeutic interventions (gait training, balance, strength, ROM, functional mobility) to maximize independence and safety.
- Reassess progress regularly, document outcomes, and adjust care plans in collaboration with the interdisciplinary team.
- Provide patient and family education on exercises, safety strategies, and activity modification to support rehabilitation goals.
- Maintain accurate electronic medical records, adhere to facility policies, and ensure compliance with regulatory and safety standards.
- Coordinate care with PT assistants and other staff; participate in care conferences and discharge planning as needed.
- Monitor for changes in patient status; report urgent issues promptly to supervisors and medical staff.

Requirements
- Active Physical Therapist license in Texas (or eligibility to practice in Texas)
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Ability to work days in a long-term care setting
- Willingness to commit to a 13-week travel assignment
- Strong communication and documentation skills; ability to collaborate within an interdisciplinary team
- Prior geriatric or LTC physical therapy experience preferred
