Title: Nurse Supervisor – Care Management / Transitional Care

Location: New York, New York

Duration: 13 Weeks (High possibility of extension)

Schedule: Monday–Friday | Day Shift

Weekly Hours: 40 Hours

Position Overview:
Seeking an experienced Nurse Supervisor to provide leadership and oversight for a Care Management and Transitional Care team. This role focuses on reducing hospital readmissions, improving patient outcomes, coordinating post-discharge care, and supervising nursing and care coordination staff. The ideal candidate will have strong leadership experience in care management, discharge planning, and utilization review.

Primary Responsibilities:

• Provide leadership and supervision to Care Managers, Care Coordinators, and support staff

• Assist with planning, organizing, directing, and evaluating nursing services

• Support transitional care initiatives to reduce 30-day hospital readmissions

• Coordinate post-discharge care plans for high-risk patients

• Develop and implement nursing policies, procedures, and quality improvement initiatives

• Monitor patient care outcomes and staff performance

• Assist with corrective action planning and performance management

• Oversee medication error reporting and quality tracking systems

• Collaborate with interdisciplinary teams to optimize community-based patient care

• Perform Director of Nursing responsibilities in their absence

• Support utilization review and discharge planning activities

Required Qualifications:

• Active RN License

• Bachelor’s Degree in Nursing (BSN)

• BLS Certification

• CCM Certification

• PRI Certification

• Minimum 7 years of nursing experience

• Minimum 5 years of supervisory or leadership experience

• Minimum 2 years of discharge planning experience

• Minimum 2 years of utilization review experience

• Strong leadership, communication, and organizational skills

Preferred Qualifications:

• Master’s Degree in Nursing

• Experience in Care Management and Transitional Care programs

• Associate Director-level leadership experience
