Clinical Pharmacist - Abdominal Transplant

Abdominal Transplant Clinical Pharmacy Specialist This position assumes responsibility for the medication management of assigned patients, monitoring and evaluating medication therapy for appropriateness based on patient health status, institutional criteria, guidelines, and cost‑efficient measures. Duties include medication reconciliation, medication monitoring, patient education, education of Pharmacy and Medical Residents, providing education to health care providers as needed, and contributing expert pharmacy content for order sets, benefit design, formularies, and other pharmacy‑related documents. Education Graduate of an accredited school or college of pharmacy Completion of a PGY‑1 Residency in Pharmacy Practice and equivalent experience (3‑5 years in Solid Organ Transplant) or completion of a PGY‑2 Residency in Solid Organ Transplant Licensed to practice pharmacy in the State of Louisiana Knowledge, Skills, and Abilities (KSAs) Computer skills and dexterity required for data entry and retrieval of patient information, proficiency with Windows‑style applications and keyboard usage. Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process. Strong leadership and teaching skills. Job Duties Provides pharmacokinetic consultation, obtains and uses appropriate laboratory data to evaluate drug selection and dosing, and communicates findings to health care practitioners. Evaluates patient response to drug therapy. Documents interventions and conducts outcome studies/evaluations to demonstrate good patient outcomes related to clinical activities. Assists with the development of pathways, best practices, and guidelines for the assigned population. Educates staff, students, patients, health care providers, and other health care professionals; submits findings for publication at least once every two years. Mentors residents, students, and new staff to meet departmental goals. Responsible for overall patient care beyond the hospital visit, through all transitions of care. Accommodates special needs of the organization by volunteering for any work/task when staffing shortages occur due to inclement weather, electrical power outages, computer downtime, or other situations. Adapts behavior to the specific patient population, including respect for privacy, method of introduction, and communication style. Performs other related duties as required. Physical and Environmental Demands Physical essential functions include frequently exerting 10 to 20 pounds of force to move objects; occasionally exerting up to 100 pounds of force.
