RN Case Manager: Discharge & Care Coordination

Cynet HealthStaff is seeking an RN Case Manager for a 13-week assignment in Oklahoma City. The role involves discharge planning, care coordination, and ensuring alignment with patient choice and regulations. Day shifts are 4x10 hours, currently from 07:30 to 18:00. Required are 2+ years as an RN and 1 year in case management, with a valid driver's license or state ID and BLS certification; CCM/ACM is preferred.
