Position Summary

The Network Performance Manager is a highly visible, externally facing leader responsible for owning relationships with a portfolio of strategically important primary care practices and driving measurable improvement in clinical, financial, utilization, and quality performance. This individual serves as a confident advisor and performance partner to physicians, practice executives, and operational leaders—translating complex data into clear priorities, leading candid performance conversations, gaining commitment to improvement plans, and maintaining accountability for agreed-upon actions and outcomes. Success requires an assertive, engaging communicator who builds credibility quickly, takes initiative, moves comfortably between relationship-building and data analysis, and respectfully challenges practices when performance or execution falls short.

Key Responsibilities

• Own the overall relationship, communication strategy, and performance outcomes for an assigned portfolio of strategically significant primary care practices.
• Establish credibility and trusted-advisor relationships with physicians, executives, clinical leaders, and administrative teams.
• Lead direct, data-informed performance conversations that identify gaps, establish priorities, and secure commitment to measurable improvement plans.
• Influence practice leaders and care teams to adopt new workflows, behaviors, and operating practices—even when Citadel does not have direct authority over implementation.
• Proactively identify performance risks, stalled initiatives, emerging concerns, and relationship issues; intervene early and recommend decisive action.
• Set clear expectations, confirm commitments, and maintain accountability for action items, milestones, and performance outcomes.
• Translate complex clinical, financial, risk, utilization, and quality data into concise, compelling recommendations tailored to the audience.
• Present performance findings and strategic recommendations to physicians, practice executives, Citadel leadership, and other key stakeholders.
• Lead practices through HEDIS improvement, MIPS, ACO participation, payer initiatives, time-sensitive incentive programs, and other value-based care priorities.
• Facilitate high-impact virtual and in-person meetings, including performance reviews, coaching conversations, collaborative problem-solving sessions, and executive presentations.
• Navigate resistance, competing priorities, and difficult conversations with confidence, professionalism, and appropriate urgency.
• Partner with practices to improve care-team design, referral management, patient engagement, EHR utilization, reporting, and other operational workflows affecting performance.
• Coordinate across internal teams, payers, and practice stakeholders to resolve barriers and accelerate results.
• Develop concise executive reporting that communicates performance, risk, recommended actions, ownership, and progress toward goals.

QualificationsEducation & Experience

• Bachelor's degree in Nursing, Public Health, Healthcare Administration, Business, or a related field required.
• Four to seven years of experience in healthcare transformation, clinical quality improvement, primary care operations, network performance, account management, or a closely related field.
• Experience working directly with primary care practices, physician leaders, or healthcare executives is strongly preferred.
• Demonstrated success, external relationships and influencing performance improvement is strongly preferred.

• Able to make sound recommendations and move initiatives forward despite ambiguity, competing priorities, or incomplete information.
• Strong analytical judgment and attention to detail, with the ability to interpret clinical, financial, utilization, risk, and quality data accurately.
• Skilled at converting detailed data into clear conclusions, compelling performance narratives, and practical next steps.
• Able to manage multiple strategic relationships and priorities in a fast-moving, mixed remote and in-person environment.
• Strong understanding of primary care delivery, patient-centered care, HEDIS, MIPS, CMS programs, ACO models, and value-based payment arrangements.
• Experience using EHRs, population health platforms, and performance-reporting tools to drive operational and clinical improvement.

Work Environment

• Twenty-five to fifty percent travel to primary care practice sites is required.
• Remote work from a home office.
• Occasional travel to Atlanta or Nashville corporate offices for meetings and events.
