Blended Case Manager

RHA Health Services, LLC

Blended Case Manager We are hiring for a Blended Case Manager in a regular position. The role supports children and adults with intellectual, developmental disabilities, mental health, and substance use challenges by utilizing community resources and services. Key Responsibilities Advocate for clients and help negotiate complex systems such as special education and social services. Assist clients in maintaining and accessing basic living needs and skills, including employment, housing, food, medical care, and recreation. Provide crisis intervention and 24/7 on‑call case management support. Develop and revise client service plans, treatment plans, and individual crisis plans. Document progress, complete assessments, new admissions, medication records, incident reports, and other required paperwork. Assist clients in securing funding and entitlements for needed services. Collaborate with county Office of Mental Health/IPAS personnel and other service providers to assess client needs. Maintain accurate case records and meet charting timeframes. Required Education and Experience Bachelor’s degree in a human services related field (e.g., sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, education) OR Associate’s degree with 12 college credits relevant to the position and two (2) years of experience in the mental health/human services field. Required Licenses and Certifications Valid driver’s license. Competencies Access community resources for clients (medical, vocational, educational, employment). Excellent verbal and written communication. Problem solving and conflict resolution. Interpersonal skills and network development. Observational skills to detect changes in client mental and physical health. Team player and collaborator with community providers. Flexibility to provide 24/7 on‑call crisis services on a rotational basis. Time management and documentation skills. Adherence to company policies and procedures. Ability to meet basic work expectations. Essential Job-Related Responsibilities Service plan development based on the client’s strengths and needs. Charting and documentation in compliance with charting requirements. Benefit acquisition and maintenance, including funding authorization and entitlement services. Participation in agency investigations. Other related duties as assigned. Pay and Bonus Pay: $20.00/hr Sign‑on Bonus: $1,000 one‑time for new hires. Additional Sign‑on Bonuses for full‑time employees: $250 after 90‑day probationary period, $250 after 6 months, $500 after 1 year. Pre‑Employment Screening Criminal background check, name checks against OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities; drug testing; education and credential verification; proof of employment history or references as required.
