Registered Nurse - Ambulatory Care/Clinic/Outpatient

Job Summary:
The Clinical Care Registered Nurse (CCRN) provides direct patient care in outpatient and home-based settings. This role requires independent application of advanced nursing skills to manage complex clinical situations and collaborate effectively with healthcare teams.

Responsibilities:

• Triage emergent and urgent situations, applying advanced nursing judgment independently.
• Monitor and coordinate care for patients with acute and chronic conditions alongside Primary Care Providers.
• Utilize care plans to deliver clinic-based and home-based nursing care, including assessment and treatment.
• Administer treatments, medications, and procedures as prescribed, ensuring patient safety and compliance.
• Conduct routine home visits to perform wound care, lab draws, assessments, and other nursing tasks.

Qualifications:

• Hold a Bachelor's degree in Nursing or an Associates degree with current RN licensure.
• Possess at least one year of experience working with elderly, frail, or long-term care populations.
• Have at least one year of home care experience and wound care expertise preferred.
• Maintain current CPR certification from AHA or willingness to obtain.
